{"id":485,"date":"2019-10-30T12:44:58","date_gmt":"2019-10-30T12:44:58","guid":{"rendered":"http:\/\/blogs.kcl.ac.uk\/clahrc-south-london\/?p=485"},"modified":"2019-10-30T12:57:30","modified_gmt":"2019-10-30T12:57:30","slug":"2nd-implementation-science-research-annual-conference-a-delegates-view","status":"publish","type":"post","link":"https:\/\/blogs.kcl.ac.uk\/clahrc-south-london\/2019\/10\/30\/2nd-implementation-science-research-annual-conference-a-delegates-view\/","title":{"rendered":"2nd Implementation Science Research Annual Conference &#8211;  a delegate&#8217;s view"},"content":{"rendered":"<p>By Andrew Walker,\u00a0 deputy clinical director, Musculoskeletal, <a href=\"https:\/\/healthinnovationnetwork.com\/\">Health Innovation Network<\/a><\/p>\n<p>The <a href=\"https:\/\/www.kcl.ac.uk\/ioppn\/depts\/hspr\/research\/cis\/about\">Centre for Implementation Science<\/a> and the capacity building theme at CLAHRC South London hosted the 2<sup>nd<\/sup> Implementation Science Research Conference on 19<sup>th<\/sup> July 2019 at King\u2019s College London, which was supported by the <a href=\"https:\/\/www.biomedcentral.com\/\">BMC,<\/a> <a href=\"https:\/\/www.ukimplementation.org.uk\/\">UK Implementation Society<\/a> and <a href=\"https:\/\/www.kingshealthpartners.org\/\">King\u2019s Health Partners<\/a>. This year, the conference\u2019s theme was \u2018Advancing the science of scaling up: Improving efficiency and effectiveness of implementation strategies in healthcare&#8217;. It attracted around 145 delegates that comprised applied-health and social researchers, health and social professionals, policymakers and service user researchers from across the globe.<\/p>\n<p>Rightly, the conference kicked off with service user perspective where Josephine tapper, a health advocate and expert patient, reminded delegates about a key purpose of any scale-up efforts \u2013 to benefit the people who use the health and social care services. In her talk <a href=\"http:\/\/www.clahrc-southlondon.nihr.ac.uk\/files\/Josephine%20Tapper.pdf\">\u2018Scaling up without leaving behind: A perspective from the \u2018shop-floor<\/a>\u2019 Josephine described her experience of being involved in the scale up of a number of interventions through voluntary work as an expert patient and offered a service user perspective on the importance of scale up and what it looks like from the \u2018doing\u2019 side.<\/p>\n<figure id=\"attachment_489\" aria-describedby=\"caption-attachment-489\" style=\"width: 242px\" class=\"wp-caption alignnone\"><a href=\"http:\/\/blogs.kcl.ac.uk\/clahrc-south-london\/files\/2019\/10\/audience.jpeg.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-489\" src=\"http:\/\/blogs.kcl.ac.uk\/clahrc-south-london\/files\/2019\/10\/audience.jpeg-242x300.jpg\" alt=\"Implementation Science Research Conference audience 2019\" width=\"242\" height=\"300\" srcset=\"https:\/\/blogs.kcl.ac.uk\/clahrc-south-london\/files\/2019\/10\/audience.jpeg-242x300.jpg 242w, https:\/\/blogs.kcl.ac.uk\/clahrc-south-london\/files\/2019\/10\/audience.jpeg.jpg 250w\" sizes=\"auto, (max-width: 242px) 100vw, 242px\" \/><\/a><figcaption id=\"caption-attachment-489\" class=\"wp-caption-text\">Implementation Science Research Conference 2019<\/figcaption><\/figure>\n<p>The conference then broke into four parallel sessions with presentations and discussions on:<\/p>\n<ul>\n<li>Understanding complexity<\/li>\n<li>Realist and qualitative evaluation<\/li>\n<li>Maternity care<\/li>\n<li>Structure, products, and exchange<\/li>\n<\/ul>\n<p>I attended the session on \u2018Structure, products, and exchange\u2019 where I and colleagues from Wessex Academic Health Science Network (AHSN) presented on our work on scaling-up innovations across local regional systems. Following the presentations, we had a lively Q&amp;A session discussion about the emotional labour involved in scaling-up effort and an evidence-base of an intervention\u2019s clinical and cost-effectiveness was just the starting point in any scale-up process.<\/p>\n<p>At lunch we had the opportunity to look at the poster presentations, which spanned a huge array of clinical specialties, interventions and settings. For example, there was work looking at scaling up guidance documents on dementia palliative care; measuring success of quality improvement in a mental health NHS Foundation Trust; lessons learnt from a novel HIV care and treatment funding\u00a0model for health facilities in Nigeria and impact of evidence-based healthcare education for Chinese medicine practitioners.<\/p>\n<figure id=\"attachment_492\" aria-describedby=\"caption-attachment-492\" style=\"width: 300px\" class=\"wp-caption alignnone\"><a href=\"http:\/\/blogs.kcl.ac.uk\/clahrc-south-london\/files\/2019\/10\/Poster-viewing.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-492\" src=\"http:\/\/blogs.kcl.ac.uk\/clahrc-south-london\/files\/2019\/10\/Poster-viewing-300x199.jpg\" alt=\"Poster viewing at Implementation Science Research Conference 2019\" width=\"300\" height=\"199\" \/><\/a><figcaption id=\"caption-attachment-492\" class=\"wp-caption-text\">Poster viewing at Implementation Science Research Conference 2019<\/figcaption><\/figure>\n<p>In the first of two afternoon plenaries, Professor Annette Boaz from Kingston University and St George\u2019s, University of London gave a provocative session asking us to consider the distinction (or not) between implementation and scale-up i.e. is scale-up simply more implementation or whether it requires a different set of approaches? Professor Boaz also challenged us to think about the ethnocentric bias in implementation studies and the risk of scaling-up as a new form of \u2018colonialism\u2019. Just because an intervention is effective for a population in a certain setting, does not necessarily mean it\u2019s suitable for all populations. Elena Pallari (King\u2019s College London, UK) talked about scaling up a QI training with urology registrar training and Posy Bidwell (Royal College of Obstetricians and Gynaecologists, UK) described the implementation of national QI project to reduce perineal trauma during childbirth. Rohina Joshi (The George Institute for Global Health, Australia) gave an elegantly simple presentation on a hugely complex implementation change on scaling up the use of verbal autopsy technology (SmartVA) to improve cause of death certification in the Philippines across &gt;700 islands, &gt;1400 municipalities. Despite the many challenges (not least the geographical barriers), Dr Joshi and colleagues have achieved amazing outcomes. When any of us feel we\u2019re having a challenging day, I\u2019d suggest looking at Dr Joshi work to help put the complexity of our problems into perspective!<\/p>\n<figure id=\"attachment_491\" aria-describedby=\"caption-attachment-491\" style=\"width: 300px\" class=\"wp-caption alignnone\"><a href=\"http:\/\/blogs.kcl.ac.uk\/clahrc-south-london\/files\/2019\/10\/Geoff-Curran-web.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-491\" src=\"http:\/\/blogs.kcl.ac.uk\/clahrc-south-london\/files\/2019\/10\/Geoff-Curran-web-300x199.jpg\" alt=\"Professor Geoff Curran\" width=\"300\" height=\"199\" \/><\/a><figcaption id=\"caption-attachment-491\" class=\"wp-caption-text\">Professor Geoff Curran, University of Arkansas for Medical Sciences (UAMS), USA<\/figcaption><\/figure>\n<p>The day closed with a plenary by Professor Geoff Curran, a medical sociologist and professor of pharmacy practice and psychiatry at the University of Arkansas for Medical Sciences (UAMS), USA.\u00a0 Professor Curran started his presentation with a quote from <a href=\"https:\/\/www.bmj.com\/content\/365\/bmj.l2068.abstract\">Greenhalgh and Papousti<\/a> (2019) that \u2018\u201c\u2026there is no simple or universally replicable way of implementing change at scale in a complex system\u201d. Rising to the challenge, he saw this as a call to action to design strong scale-up studies to understand what does and doesn\u2019t works.
